Registration Information
Registration Information:
Students new to Hillsborough County School District must provide all of the following documents required for enrollment:

  • Final Report Card from previous school
  • An original/certified Birth Certificate
  • School Physical / Student Health Exam
  • Certificate of Immunizations
  • Proof of Residency
  • Student’s Social Security Card

Final Report Card –
  • Report card MUST SHOW promotion to 6th, 7th or 8th grade.
School Physical / Student Health Exam -
  • School physical/Student Health Exam Form is available from your child’s doctor or any walk-in clinic.
  • Health exam MUST BE LESS THAN ONE YEAR OLD as of the 1st day of school
  • School physical/Student Health Exam form MUST HAVE doctor’s signature.
Proof of Residency –

Please note that proof of residency requirements HAVE CHANGED!

Students currently enrolled in Hillsborough County Public Schools District that reside within the Martinez Middle School boundary MUST PROVIDE two (2) FORMS OF PROOF OF RESIDENCY.

Examples of proof of residency are as follows:
  • TECO Electric bill (dated within the last 3 months).
  • A lease agreement showing the name of the student's parent/guardian and the address of the rental property.
  • A Hillsborough County Tax receipt showing Homestead Exemption.
  • A Warranty Deed or notarized home purchase contract or Real Estate Closing Statement showing the name of the parent/guardian of the student and the address of the home.
  • A valid state-issued driver’s license showing your current address.
  • A valid Voter's Registration Card showing your current address.
Immunization Requirements-

PLEASE NOTE THAT THE REQUIRMENTS HAVE CHANGED!  

2019–2020 School Entry Requirements
Before attending school in Florida (kindergarten through 12th grade), each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:


Public/Non-Public Schools Kindergarten through 12th Grade:
Additional Seventh Grade Requirements:
 In addition to kindergarten through 12th grade requirements, students must have the following vaccinations:
 Four or five doses of diphtheria-tetanus-pertussis (DTaP) vaccine
 One dose of tetanus-diphtheria-pertussis (Tdap) vaccine in grades seven through twelve
 Two or three doses of hepatitis B (Hep B) vaccine
 An updated DH 680 form to include Tdap, must be obtained for submission to the school
 Three, four, or five doses of polio vaccine*
 Two doses of measles-mumps-rubella (MMR) vaccine
 Two doses of varicella vaccine† for kindergarten and grades one      through seven
One dose of varicella vaccine† for grades eight through twelve
* If the fourth dose of vaccine is administered prior to the fourth birthday, a fifth dose of polio vaccine is required for kindergarten entry only.
†Varicella vaccine is not required if varicella disease is documented by the health care provider.
